Make a Referral
AnchorPointVA partners with DSS social workers, TFC case managers, and authorized professionals to provide structured, supportive transitional services for youth transitioning into adulthood.
Our referral process is designed to be clear, collaborative, and youth-centered, ensuring the best possible alignment between program services and each young person’s needs.
Who Can Submit a Referral
Referrals may be submitted by:

DSS social workers

TFC case workers
-
Authorized DSS representatives
All referred youth must be enrolled in the DSS Fostering Futures program and meet eligibility requirements prior to acceptance.

How the Referral Process Works
Step 1: Submit a Referral
Authorized DSS or TFC professionals complete a secure online referral form with basic youth and case information to initiate the intake process.
Step 2: Referral Review
After submission, a team member from AnchorPointVA will review the referral to ensure program alignment, capacity, and readiness for independent living support.
Step 3: Application & Youth Questionnaire
If the referral meets initial criteria, an application package and youth questionnaire are sent to both the referring professional and the youth for collaborative completion.
Step 4: Coordination & Next Steps
Once materials are reviewed, AnchorPointVA coordinates next steps directly with the referring professional and youth to determine program entry and timeline.

- Agency
- Email address
- Phone number
Youth Profile
- Name and contact information
- Age
- School or employment status
- Anticipated timeline for services
Strengths & Support Needs
- Areas of success
- Areas of struggle (physical and emotional)
Health & Wellness
- Medication needs
- Therapy participation
- Physical or emotional health considerations
Placement History
- Number of previous placements
- Total time in foster care
